If you believe you have experienced discrimination and would like to file a discrimination complaint, you will need to fill out a complaint form.
There are three ways to obtain a complaint form:
- contact the Commission in writing
- contact the Commission by telephone
- complete the Commission’s complaint assessment tool, an online questionnaire that will help you determine if you have a valid complaint.
It will take approximately 30 minutes to complete your online assessment. Contacting the Commission by phone or in writing may mean you will have to wait longer to find out if your complaint is valid.
Once you have confirmed that you have a valid discrimination complaint, and are ready to fill out your complaint form, make sure to include:
- the action or decision that you think is a discriminatory practice,
- the grounds of discrimination involved, and
- how the discriminatory practice affected you.
