DOCUMENTATION CHECKLIST

The following is a checklist of all the documentation that would be useful in assessing compliance. Please feel free to provide any other documents that you consider pertinent or that could be useful to the compliance review.

Please check the box next to any documents you have enclosed

Requirement #1

 A) A copy of the self-identification survey package used at the time of the last survey of the whole workforce as well as any related materials
 B) A copy of the current self-identification package used with new employees
    C) A description of:

 The survey process including any lead-up or follow-up strategies
 The process by which information is updated in the HR system.

Requirement #2

 D) The workforce analysis report and relevant explanations

Requirement #3

 E) The report stemming from the employment systems review and relevant explanations
 F) A description of the process for the review of new policies and procedures

Requirement #4

 G) The current and previous Employment Equity Plans
 H) Any policy (or policies) and procedures related to accommodation
     I) The policy (or policies) and procedures related to:

 Harassment/discrimination in the workplace
 Employment Equity
 Recruitment/Promotion/Training/Termination

 J) The short-term hiring, promotion and representation goals
 K) A description of different factors that you have taken into consideration in determining your hiring/promotion goals
 L) The long-term goals to increase representation

Requirement #5

 M) The monitoring strategy for the Employment Equity Plan
 N) The monitoring reports on the Employment Equity Plan
 O) Documents pertaining to how managers are held accountable for implementing employment equity (e.g.: copies of managers’ performance reviews and objectives related to employment equity)

Requirement #6

 P) The updated short-term numerical goals for hiring, promotion, and representation; the rationale for the updates; and the factors considered in setting the new goal
 Q) The amendments made to the Employment Equity Plan and the rationale for these amendments

Requirement #7

 R) The communication strategy related to employment equity, accommodation and accessibility
    S) Copies of information provided to the workforce which describes:

 - the purpose of employment equity
 - the measures taken or planned to implement employment equity
 - the employer’s progress in implementing employment equity

These can include copies of brochures, Internet site, Intranet site, posters, e-mails, etc.

Requirement #8

 T) The consultation strategy for the development and implementation of the Employment Equity Plan and documents showing that consultation occurred

Requirement #9

 U) A description of how employment equity records are maintained
